Review & Authorize

I understand that the Standard Membership is month-to-month and automatically renews. I authorize Silk Lash Empire to charge $165 once per month until I cancel future renewal.

I understand that the Select Membership is month-to-month and automatically renews. I authorize Silk Lash Empire to charge $185 once per month until I cancel future renewal.

I understand that the VIP Membership is month-to-month and automatically renews. I authorize Silk Lash Empire to charge $195 once per month until I cancel future renewal.

I understand that monthly SLE Credits are provided as part of my paid membership benefits, do not roll over, and unused monthly Credits expire/reset at the end of the applicable paid billing cycle.

I have reviewed and agree to the current Silk Lash Empire Membership Terms & Conditions and understand the recurring billing, membership benefits, SLE Credit rules, cancellation terms, and applicable Silk Lash Empire Salon Policies.

I understand that I may cancel future membership renewal by calling (818) 849-6976, emailing salon@silklashempire.com, or texting (818) 298-4274, subject to the current Membership Terms & Conditions.
